What's in the NGI reimbursement form

The form has two sections plus an office-use page. Section A is the policyholder's part: policy or card number, policyholder and employee details, contact information, and the total claim amount, signed as a declaration authorizing NGI to obtain medical records related to the claim. Section B is completed by the treating physician: patient details, diagnosis, ICD code, presenting complaints, duration and onset of the condition, relevant medical history, and, for hospitalization, admission and discharge dates, procedures, and CPT codes.

There's no separate NGI reimbursement form for dental treatment or for provider-submitted claims. One form covers out-of-network treatment of any kind.

Common reasons NGI reimbursement forms get rejected

  • Submitting past the deadline. NGI requires reimbursement claims within 30 days of treatment in the UAE, or 90 days if treated abroad.
